Founded in 1993, CommunityHealth is dedicated to serving the uninsured and underserved in Chicago and surrounding communities. Last year, CommunityHealth provided medical and dental visits for more than 10,000 patients. This level of care is made possible thanks to the time and talent of more than 1,000 volunteers, including 350+ providers (physicians, nurses, nurse practitioners, dentists, pharmacists, counselors, etc.) who volunteer over 45,000 hours of their time every year.
CommunityHealth is located in Chicago’s West Town — 2611 W. Chicago Ave.
As the largest volunteer-based free medical facility in the nation, we offer demonstrated success and an innovative model for providing compassionate
and comprehensive medical and dental care to the uninsured.
As the largest free clinic in the nation, CommunityHealth acts as a convener of community leaders from many sectors to discuss solutions for providing accessible health care for all Chicago residents. We fully recognize that providing people with the opportunity to be healthy allows them to be better parents, employees, neighbors, and citizens. Together, we can deliver a healthier Chicago.
