The mission of CommunityHealth is to serve people without essential health care. Our vision is quality health care for all.

​

CommunityHealth is one of the largest volunteer-based health centers in the nation. We provide primary and specialty care, medications, lab testing and vaccinations, mental health services, and health education at no charge to low-income, uninsured and underinsured adults in Chicago.

​

In 2021, we opened our first microsite clinic, a model that co-locates primary care services within an existing social service agency in a neighborhood where our patients live and work. This unique model offers critical access in high-need communities, and helps to eliminate barriers (like transportation and child care) that might otherwise prevent individuals from seeking care. Because of this model, we are able to bring high quality health care closer to our patients’ homes.

We are currently operating three successful microsites, and preparing for the fourth: CommunityHealth at Rincon Family Services. With the AbbVie Foundation’s support of our microsite model, we are able to bring care closer to home for many underserved Chicago residents.
